- Click this link (at the bottom of the page) to view and print the waybill and the contract.
- You will want 3 copies one for your records, one for our records, and one for the customers records.
- Fill it out with all of yours and customers information.
- Give it to the driver who picks up the pet
- You will also need (2)two copies of the health certificate no less than 10 days old.
- If the dog is over 3 months it will need a rabies shot record.
- The pet will need a plastic ID collar with Shippers name, ID number and Consignees name and ID # or confirmation # on it.(You can get a collar from your veterinarian or kennel supply)
- The pet will a crate sticker with city and state it is traveling to, ID number and shippers name (A mailing address sticker will work)
- Put all papers including 1 one health certificate to the consignee in the envelope, leave waybill and 1 one copy of health certificate out and paper clipped together.
- If you are sending food or anything else with your pet make sure it is clearly marked on the waybill and the bag with the city and stat it is going to and the consignees name and the shippers name
- This is important to have the paper work ready because it helps expedite the shipping process.
